Our Benefits
Medical
Argon ST currently offers three health plans administered by United Healthcare and one health plan administered by Kaiser to meet the varied medical needs of our employees. To locate a network provider near you, visit www.uhc.com or www.experiencekp.org .
- The UHC Choice Plan (not available to employees at our MI or PA facilities) provides benefits for in-network services only. The plan offers a national network of providers. You are not required to choose a Primary Care Physician. Referrals are not required for visits to Specialists. Regular full-time employees pay $16.00 per pay period for individual coverage, $27.00 for employee/child(ren) coverage, $42.00 for employee/spouse coverage, or $55 for family coverage under the Choice plan.
- The UHC Preferred Provider Organization Plan (Options PPO) provides coverage for care received from both in-network and out-of-network providers. Like the Choice Plan, you are not required to choose a Primary Care Physician and referrals are not required for visits to Specialists. The Options PPO also offers a network that is nationwide. Regular full-time employees pay $30.00 per pay period for individual coverage, $53.00 for employee/child(ren) coverage, $70.00 for employee/spouse coverage, or $96 for family coverage under the Options plan.
- The UHC Passive PPO Plan (available to employees at MI and PA facilities only) provides the same coverage level for both in and out of network services. Referrals are not necessary and you are not required to choose a Primary Care Physician. Regular full-time employees pay $30.00 per pay period for individual coverage, $53.00 for employee/child(ren) coverage, $70.00 for employee/spouse coverage, or $96 for family coverage under the Passive PPO plan.
- The Kaiser Permanente Plan (available to employees in CA facilities only) provides HMO coverage for in-network services only through a comprehensive managed-care environment. Referrals are required. Regular full-time employees pay $23.00 per pay period for individual coverage, $33.00 for employee/child(ren) coverage, $50.00 for employee/spouse coverage, or $67 for family coverage under the Kaiser plan.
Dental
Employee and Dependent dental coverage is offered through United Healthcare. Argon ST offers a PPO dental plan. The cost to regular full-time employees for individual coverage is $4 per pay period. If a regular full-time employee chooses dependent coverage, the employee pays $7 per pay period for employee/spouse coverage, $9 per pay period for employee/child(ren), and $13 per pay period for family coverage.
Medical FSA
Argon ST maintains a Medical FSA. The plan provides full-time plan participants and their spouse and children the opportunity to be reimbursed up to $2,000 ($1,500 for PT30; $1,000 for PT20) per plan year for medical, vision, and dental expenses actually incurred, but not covered or paid for by insurance or other reimbursement plans. Employees do not have to enroll with the company's health or dental plan to be eligible. Benefits are prorated for your first year of employment.
This plan also allows participants to add their own pre-tax contributions (up to $3,000 per year) to Argon ST's contributions. The ability to save more in the account could help employees plan for larger eligible medical and dental procedures in a given plan year. Expenses will be charged against the employer money first and then the employee money.
During your first week of employment and at annual open enrollment times, you decide how much money, if any, you want to deposit in an account for the year. The maximum allowable benefit is $5,000 with $2,000 contributed by Argon ST and up to $3,000 allowable for you to contribute. The amount is divided evenly among the number of pay periods in a calendar year (26). Money is then deducted from your paycheck on a pre-tax basis. Contributions to FSA's reduce the amount of taxable income. This results in savings of FICM, FICA, federal and state income taxes on the amount of salary deferral.
Life and Disability Insurance
Argon ST pays the full premium for both Life and Disability insurance for each employee scheduled to work at least 20 hours per week.
Life and Accidental Death & Dismemberment (AD&D) Insurance
Argon ST provides basic life insurance in the amount of 2.5 times an eligible employee's annual salary to a maximum or $250,000. An additional Accidental Death and Dismemberment (AD&D) benefit for the same amount applies to death or specific injury resulting from an accident. This benefit is company paid.
Short-Term Disability Insurance
Eligible employees receive 66 2/3% of their monthly earnings to a maximum benefit of $1,000 per week. Benefits begin after 7 days of injury or illness. This benefit is company paid.
Long-Term Disability Insurance
Eligible employees receive 60% of their monthly earnings to maximum benefit of $6,000 per month. The elimination period is 90 days; or the date your short-term disability payments end, if applicable. This benefit is company paid.
Voluntary Life/AD&D Insurance
Argon ST provides the opportunity for employees to purchase additional Life/AD&D insurance for themselves and their dependents at the group discounted price. Unimerica is the carrier for Voluntary Life/AD&D.
Enrollees must complete an Evidence of Insurability (EOI) Form for any Voluntary Employee Life, Spouse Life, and Child Life. New hires have Guaranteed Issue for $100,000 for employee life and $25,000 for spouse life. AD&D coverage is always guaranteed up to 5 times the annual salary up to $500,000 (up to $10,000 for Child AD&D) Premiums are paid through convenient payroll deduction.
Fitness Program
*Note: this benefit varies by operating unit.
Subject to prior management approval, Argon ST will reimburse full-time employees up to $200 per year to enroll in a formal fitness program. This amount can be applied toward fitness club membership fees, specific exercise programs, or the purchase of exercise equipment.
